Wound Care Management

Wound Care is an all too common problem for senior citizens. There are many factors to consider in the risk and cause of wounds and pressure ulcers. Oftentimes, wounds are overlooked and thought to be superficial, when in fact they could become extremely dangerous and very painful.

At Deer Meadows Home Health & Support Services (DMHHSS), we understand the frustration and discomfort of dealing with a wound. Managing the pain, the medications, and the dressings can be a seemingly endless task. Depending on your condition or where the wound is located, dressing a wound may not even be physically possible for you. For these reasons, we proudly offer personalized home wound management programs to help your wound heal, and get you back on your feet again.

DMHHSS has a specialized wound care team available for individuals who need assistance with healing complicated wounds at home. Our team of dedicated staff members are specially trained in the latest wound care technology, and up-to-date in the newest medications and treatments available to help your wounds heal.

Patients admitted to our wound program are assigned a primary nurse who makes regular visits to your home. While treating and monitoring your wound, our staff also educates caregivers on the best practices to help improve your healing, including cleanliness of the wound, nutritional needs, and environmental hazards. We will regularly review your plan of care to ensure that your needs are being met. We will also work closely with and keep your physician informed on your healing progress.

We offer a team of Wound Care Certified (WCC) Registered Nurses who are responsible for planning and providing care for patients with complicated wounds, pressure ulcers, and/or any patient who has a potential skin care problem, in order to assist the patient in achieving a maximum level of wellness and independence. The WCC will consult with other members of the health care team in the management of acute and chronic wounds, and will be actively involved in the management and prevention of pressure ulcers. We will take the necessary steps to ensure you are provided with care that exceeds expectations.

Wound care management can also be vitally important to diabetes sufferers, who often find that, due to altered blood glucose levels, the wound healing process can be a long and sometimes painful one. The trained professionals from DMHHSS will do everything possible to help you or your loved one recover quickly in the comfort of your home. We will provide education on the symptoms of infection, the use of specialized equipment, and outline an exercise and nutritional program for proper recovery.
