We offer a team of Wound Care Certified (WCC) Registered Nurses who are responsible for planning and providing care for patients with complicated wounds, pressure ulcers, and/or any patient who has a potential skin care problem, in order to assist the patient in achieving a maximum level of wellness and independence. The WCC will consult with other members of the health care team in the management of acute and chronic wounds, and will be actively involved in the management and prevention of pressure ulcers. We will take the necessary steps to ensure you are provided with care that exceeds expectations.
Wound care management can also be vitally important to diabetes sufferers, who often find that, due to altered blood glucose levels, the wound healing process can be a long and sometimes painful one. Learn more about how our home diabetes management services and chronic wound care services often go hand-in-hand.
The trained professionals from DMHHSS will do everything possible to help you or your loved one recover quickly in the comfort of your home. We will provide education on the symptoms of infection, the use of specialized equipment, and outline an exercise and nutritional program for proper recovery. Our goal is to provide greater independence and preserve the patient’s dignity.
Our in-home care registered nurses not only specialize in the care of patients with wounds, we have skilled nurses available for other acute and chronic conditions, including: Congestive Heart Failure, COPD, Diabetes, After Surgery Care, Depression and Hospice Services.
