Deer Meadows Home Health and Support Services, LLC Awarded CHAP Disease Program Certification

PHILADELPHIA, PA — December 2, 2022 — Community Health Accreditation Partner, Inc., (CHAP) announced today that Deer Meadows Home Health and Support Services, LLC, (DMHHSS), under the direction of DMHHSS Administrator Stanley Rynkiewicz, has been awarded a Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Wound Care and Diabetes Disease Program Certification under the CHAP Disease Program Standards. Deer Meadows Home Health and Support Services, LLC is the first Home Care Agency in the United States to be awarded this Multiple Disease Process Certification through CHAP.

CHAP certification demonstrates that Deer Meadows Home Health and Support Services, LLC meets the industry’s highest nationally recognized standards. The rigorous program evaluation by CHAP focuses on program structure and function, quality metrics, advanced education for staff, and demonstrated self-management tools for patients.

On 7/25/18, CMS Home Health compare website updated its quarterly data. Once again, Deer Meadows Home Health and Support Services, LLC (DMHHSS) is rated 5 Stars. Since CMS launched Home Health Compare Website July 2015, DMHHSS has been rate 5 Stars ten out of eleven rating periods. The only exception was October 2015 when DMHHSS was rated 4.5 Stars. This is an outstanding achievement and join me in congratulating the DMHHSS staff on their excellent quality of patient care and customer service. This is star rating is evidenced by our patient’s quality outcomes and satisfaction surveys which are publicly reported here on this website. We are one of the 10 agencies rated 5 Stars this quarter in PA. This is an AWESOME ACCOMPLISHMENT. Our Philadelphia Home Care Agency Receives DHS Award!

The Department of Human Services recently announced the awarding of nearly $8 million dollars in grant money to member agencies of the Pennsylvania Homecare Agency (PHA) to connect their Electronic Health Records (EHRs) to the statewide health information exchange. Deer Meadows Home Health and Support Services was one of only six home care and hospice agencies in the state of Pennsylvania to receive the grant. Our Philadelphia home care agency will use the funding to upgrade our software programs and to provide training to other staff members. The funding will also enable Deer Meadows Home Health and Support Services to integrate into the Keystone Health Information Exchange (KeyHIE), thus providing real-time notifications from over 20 hospitals in the Philadelphia area about patient admissions, discharges, and hospitalizations. We will be able to better coordinate care of our patients with other health care providers, in turn improving outcomes for our patients.

Lieutenant Governor Michael J Stack III Congratulates Deer Meadows Home Health and Support Services

Deer Meadows Home Health and Support Services, LLC (DMHHSS)’s representatives (Stanley Rynkiewicz, Administrator, Irene Dudley, Clinical Director, Gina Grosh, Oasis/Coding Manager, Kathleen Liggins, Private Duty Manager, Riley Roberts Marketing and Sales Director and Matthew Mortka, Community Liaison) met with Lieutenant Governor Michael J Stack III in the Lieutenant Governor’s Parlor Suite in Harrisburg at the State Capitol on April 25, 2017.

During this time, Lieutenant Governor Michael J Stack III congratulated DMHHSS our 5 Star CMS Home Health Compare rating. We also discussed home health care, changes in managed Long Term Support Services, (MLTSS), and the future of home health care in the State of PA.

On January 20, 2015, Mike Stack was sworn in as Pennsylvania’s 33rd lieutenant governor, and in that role serves as presiding officer of the Pennsylvania Senate. He also chairs the Board of Pardons and was appointed by Governor Wolf to chair the Pennsylvania Emergency Management Council.

Lt. Gov. Mike Stack is a former Captain in the Pennsylvania Army National Guard and a graduate of the U.S. Armed Forces Officers Basic Course. He also served as a Judge Advocate General (JAG) for the 28th Infantry Division. Before being elected as lieutenant governor, he served as a member of the Pennsylvania State Senate representing the 5th District, which includes Northeast Philadelphia, Brides burg, and portions of Port Richmond and Juanita Park, from 2001 to 2015. As a senator, Lt. Gov. Stack was the Democratic chair of the Banking and Insurance Committee and a member of the Aging and Youth, Communications and Technology, Judiciary, Local Government, and Policy committees.

The picture below was taken in the Lieutenant Governor’s Private Office. The backdrop of this photo is the main architectural feature of the private office. It is a fireplace with a rose red Numidian North African Marble surround and mantel. Above the mantel, rising to the height of the ceiling, is a Palladian-inspired over-mantel made of mahogany, containing a custom clock with a dial of the Pennsylvania Coat of Arms in polished brass. The remaining portion of the wall contains portraits of past lieutenant governors.

Deer Meadows Home Health and Support Services​ Administrator, PHA Board Member, and PHA Lobby Day Team 43 Leader, Stanley Rynkiewicz RN, MSN, WCC, DWC, NHA brought his team to PHA’s Pre Lobby Day Dinner Monday April 24, 2017 at Duke’s Restaurant in Wormleysburg, PA. It was awesome to see State Representatives in attendance representing their districts to speak to the home care leaders in their support of the home care industry. The Representatives present from Deer Meadows Home Health and Support Services​ were State Representative Mary Jo Daley (D-148) Montgomery County, State Representative Pamela Delissio (D-194) Montgomery County, State Representative Michael Driscoll (D-173) Philadelphia County, and State Representative Eric Roe (R-158) Chester County. Forum Honoring Direct Care Worker Nominees for the 2017 Pennsylvania Direct Care Worker of the Year Award

On Tuesday, April 4th Kate Liggins, Private Duty Manager, and Diane Scott, full time Certified Nursing Assistant, attended a forum honoring the 164 direct care workers that were nominated by their organization as the 2017 Pennsylvania Direct Care Worker of the Year Award. All nominees were invited to the Forum today, all expenses paid, for lunch, training and the presentation of the award by Secretary of Aging Teresa Osborne.

This unique event, which is sponsored by the Department of Aging, PHA, and Preferred Benefit Consultants, featured lunch and training and ended with the surprise announcement of direct care worker of the year. Guest speakers included Leigh Weintraub, an innovative pioneer in the world of personal transformation and moving people toward their peak performance and also Terrell Cannon, Director of Workforce Development, Home Care Associates who presented on professionalism, ethics and boundaries. Diane Scott was Deer Meadows Home Health and Support Services nominee. Diane has been working for Deer Meadows Home Health Support Services since 2009 and has played a huge role in the success here at deer meadows. Diane’s provides top notch care to each patient she comes into contact with and although Diane was not chosen as 2017 Pennsylvania Direct Care Worker of the Year Award, Diane will always be a superior provider of care to her patients and will continue to do so as she grows with Deer Meadows Home Health Support Services.

Today, October 19, 2016, CMS Home Health compare website updated its quarterly data (see below screen shots for details). Once again Deer Meadows Home Health and Support Services, LLC (DMHHSS) is rated 5 Stars.

Since CMS launched Home Health Compare Website last July DMHHSS has been rate 5 Stars four out of five rating periods. The only exception was last October when DMHHSS was rated 4.5 Stars. This is an outstanding achievement!

This is star rating is evidenced by our patient’s quality outcomes and satisfaction surveys which are publicly reported here on this website. This rating period only 1 % of the Home Health Agencies in Pennsylvania received a 5 Star rating (see below screen shot for details).

Also, only 4% of the Home Health Agencies Nationwide received a 5 Star rating (see below screen shot for details). This is an AWESOME ACCOMPLISHMENT.
